fix: replace deprecated substr() and fix MPV IPC timeout handling

- Use manual time calculation instead of fragile toISOString().substr()
- Explicitly resolve null on MPV IPC timeout to prevent hanging promises
- Clear timeout handle in all event paths to prevent races
